I guest your one is Phalaenopsis, it may be too much water due to the moss kept water around root for most of the time . Root needs breathing . You may take away the moss and clear out all rotten root and try again, it will grow again. Good luck.
Yes, I think it is too much moisture since you are put moss on top of it. Don't use 100% moss to wrap around the root...this is no good for the root as the root needs to breath. For my orchids, I don't use moss at all, but nothing wrong to use, but only a bit n a thin layer on top. Also, I suggest to use some pot that can breath, such as clay pot without paint outside.
